Community Health Awards

The MetroWest Health Foundation periodically invites nominations for its annual Deborah Blumer Community Health Leadership Award. The award is designed to recognize an individual or organization that has demonstrated outstanding leadership in improving health and/or health care in the MetroWest area. The recipient of the 2012 award will be announced in late March.

Award Criteria

Nominee should be directly responsible for advancing the health of the community through their actions to:

  1. mobilize individuals, resources and organizations to confront an important community health issue;
  2. develop new services and supports to improve the health of vulnerable populations; or
  3. raise the level of awareness of an important health issue in the MetroWest area.

Qualifications

Nominees must work or provide services to individuals in one or more of the 25 towns in the Foundation's service area.

Selection Process

A committee will review all nominations and submit recommendations to the Board of Trustees of the Foundation for final approval.

Award recipients will be expected to attend the Foundation's annual dinner meeting to accept their award.

Trustees, committee and panel members, and staff of the MetroWest Health Foundation are not eligible for nomination.

Nominate Your Hero
