Requests for proposals are issued in the spring and fall of each year. Before getting started, be sure to review the Applicant & Grantee Guide.

Priority Areas

  • Girls' Mental Health
    The foundation's grants for girls' mental health are designed to promote healthy lifestyles and environments that enable girls to manage stress, anxiety and depression.
  • Improving Health Equity
    The foundation will support initiatives focused on decreasing health disparities in the following areas: maternal health outcomes; access to primary and specialty care; vaccine equity; and chronic disease prevention.
  • Health and Housing Stability
    The foundation will support initiatives that seek to decrease housing insecurity and increase support services to older adults, families with children, and newly arrived immigrants.
  • Innovative Action to Address Community Health Priorities
    The foundation will invest in the planning, development and implementation of solutions to the health challenges prioritized in the 2023 Community Health Assessment.

Grant initiatives are subject to change. Availability of the grants and the due date for proposals will be announced via our e-newsletter.

Proposal Guidelines

Please read the Fall 2024 Grant Guidelines. The proposal deadline is 4:00 p.m. on Friday, October 18, 2024. Incomplete or late applications will not be considered. Applications that are in-progress at 4:00 p.m. on the day of the deadline will not be considered.

Concept Papers

The foundation requires applicants to submit a one-page concept paper and cover sheet plus a logic model prior to a full proposal. Concept papers help the foundation assess whether the proposed project is aligned with its funding priorities. Concept papers must be submitted by Friday, October 4. The foundation strongly encourages applicants to submit their concept papers in advance of this deadline to allow for timely feedback.

Steps to Apply

Applications must be submitted online. Please follow the steps listed below. You will be asked to submit general information about your organization and project; a budget and budget narrative; narrative detailing your project; and supporting documents.

If you are a New Applicant, once you setup your account information return to this page and follow the steps below to access and submit your application.

Step 1

Download and complete the forms listed and save them to your computer in order to attach them to the online application.

Step 2

On page 2 of the online application, you will be asked to write a brief abstract of your proposal that summarizes the project including target audience and goals. We suggest that you prepare the abstract prior to proceeding with the online application and save this information to your computer. You will then be able to cut and paste this information from your word processor into the box provided on page 2 of the online application. Please keep your response to 150 words or less. Sample abstract.

Step 3

On Page 3 of the application you will be asked to upload the following supporting documents, if applicable, to your online application. Documents should be in Word, Excel or PDF format. Do not upload files with JPEG, JPG, or TIF extensions. Documents will NOT be accepted via U.S. mail, email or fax.

List of your organization's current Board of Directors

List the name of each director, town of residence and affiliation only. Not applicable for municipal agencies. Do not include contact information such as emails and phone numbers.

Financial Statements

Financial Statements for your organization's most recent completed fiscal year, including balance sheet and income and expense statement. 
If your organization is audited, please provide a complete set of audited financial statements. Otherwise, submit a copy of your 990. Not applicable for municipal agencies.

Letters of Collaboration

Letters of Collaboration are not required, but highly recommended. The letters should be signed and specifically state the collaborator's individual or organizational contribution to the project. Replicated form letters from multiple organizations are strongly discouraged. Multiple letters must be scanned into one document.

Note: Proposals involving schools must include a letter of collaboration from the superintendent.

Step 4

Complete the online application. The online application consists of four areas:

  1. Applicant Login
  2. General Organizational/Project Information Page
  3. Abstract
  4. Attachment Upload Page

Important Notes: Applicants must create an account in order to access the application and save their work for completion at a later time. This user account and password will allow you to submit your online application and your grant reports, if your project is funded. Therefore, we highly recommend that your account information and grant application are created and submitted by the project director or development director. You will receive an email with a link to your application and confirming your password.

Concept Paper

A Concept Paper must be submitted and approved prior to submitting a full proposal.
