The 2023 Community Health Assessment survey is now open and we want to hear from you! There are no right or wrong answers – it is your opinions and experiences that matter. The 10-minute, anonymous survey can be completed online using one of the following links:

What is a Community Health Assessment? 

A Community Health Assessment is an effort to identify strengths and challenges in the region related to health and accessing health care. The information gathered from this process will be used to inform future health programming and services for a range of organizations in the MetroWest region. This year’s health assessment will include a review of health data, key informant interviews, focus groups, and community surveys.

CHAs have been conducted in our region every three years since 2013. The last CHA was completed in 2019.

What kind of information will you ask me in the community survey?

Questions in the survey ask about your experience in accessing healthcare and important community resources, as well as your opinions on the health of your community. We also ask for some demographic data to understand whether people's experiences with health and the health care system vary based on their personal characteristics.

You can choose to skip any questions if you don’t feel comfortable answering.

What will happen to my data and responses?

Your responses to the community survey are anonymous and data will be reported in aggregate so individuals cannot be identified. The information gathered from this process will be used to inform future health programming and services for a range of organizations in the MetroWest region. 

It is our goal to have a finished product to share with the region in early fall. The finished product will be available on our website mwhealth.org

Who is involved?

The MetroWest Health Foundation is leading this year's Community Health Assessment and is partnering with other organizations to reach as many residents as possible. We are seeking feedback from people who live in the cities and towns listed here (the foundation’s catchment area) via the Community Health Assessment Community survey.

Please spread the word! Encourage your neighbors and friends to complete the survey. An outreach toolkit is available here.

I work with the public. Should I share it with them?

Yes! We want representation from all 25 cities and towns. And, the surveys can be completed multiple times from the same computer - perfect for senior centers, libraries, or other public spaces.
